Contract

For the Intern

  1. I will arrive on time to internship.
  2. I will have my time card signed by my mentor every day.
  3. I will go to internship every day that I am scheduled, except in the case of emergency or illness.
  4. In case of an emergency or illness, I will speak to my internship instructor AND call my mentor no later than 10 a.m. to inform them.
  5. If I am absent from school, I will email my internship instructor AND call my mentor by 10 a.m. to inform them.
  6. I will make up any days or hours I miss at my internship.
  7. I will dress appropriately for work.
  8. I will approach work with a good attitude.
  9. I will conduct myself appropriately at all times.
  10. I will communicate with my instructor and my mentor about any problems or difficulties that I have at internship.
  11. I will work hard to use this internship opportunity as a learning experience.

For the Mentor

I will ensure that the intern lives up to his/her responsibility to this organization, and at the same time I will provide the intern with valuable experiences from which to learn. I will promptly inform my intern’s International Academy internship instructor of any problems, including any absences, tardiness, and conduct or attitude issues. I will only sign time cards with the accurate dates and times already filled in.

For the Internship Instructor

I will offer support to the intern, communicate regularly with the internship mentor, and mediate any problems or difficulties that may arise.
